File format for md5($pass.$salt)

I'd like to use to hash type "MD5_PS" (i have the salt key and the final hash).
The following wiki page indicates that this is supported (, but it doesn't give the file format you should use ("I honestly forget what the file format is, so I'll fill this out later." on the wiki :( )

Any idea of the file format I should provide to Multiforcers to indicate both hash and salt key?
